Wow! He really swings, doesn't he?

I know its really low dosing, and the duration seems short for him (steep curves often) so I'm wondering if you'd ever thought of any of the following to try for smoother sailing (these are ideas, not mandates!):
1) getting a Humulin R compounded to a lower concentration (maybe U-50 or U-25) so you could give him 0.05 - 0.25 units when he was really high (its hard to eyeball in a u-100 syringe otherwise)
2) attempting split doses (it'd be hard with such small amounts, though not impossible) - maybe review how Momma of Muse is doing with Musette
3) TID (every 8 hours dosing) - again it'd be tiny doses
4) a switch to Levemir - still tiny, but more overlap, so maybe smoother curves - perhaps someone could let you try some before committing

The only other thing I can think of - if you rotate varieties of cat food, with slightly differing carb values, maybe he's really, really, carb sensitive and even a 1% change affects his glucose. Spitzer reacts like that - you can see it on a recent test 4 days ago.

I would try cutting the dose in half before any of those advance techniques. Although both that and a switch to Levemir have been mentioned in previous threads. With .5 U of Lantus causing a drop like that, supplementing with R would be highly dangerous, even if it was diluted.

Split dosing at .25U each time 2 hours apart the way Mel is doing might be helpful though and prevent those huge spikes. When is the next time you can get a more complete curve?

Forget the R; there is no need to use R at such a low dose.

Better to try a .25u dose and let the numbers settle, even if the flat curve is a bit higher..... it's easier on the cat.
You could play around with foods to manipulate the curve.... just need to get a fresh curve and see where the highs and lows are.... be sure to note times and amounts of foods so that you can see how the numbers were affected.

Lev would be another option; Shadoe likes Lev much more than Lantus when she was more tense.

Wow his spreadsheet does look a lot like Musette's for those wild swings. I would be leery about using R too just from the way he can nose dive on .5u but then again that was exactly what I was dealing with, with Musette.

She would do that same fly way up into the red and black numbers as well as drop clear down into the 30s, so far since I have been split dosing her she has levelled out quite a bit and while she still bounces off normal numbers some even those are getting less high and she isn't falling nearly as steeply. She also still food spikes but the split-dosing seems to be helping with that as well, that second dose seems to help take the edge of those wild swings.

If you decide to try split-dosing and have any questions I will try to answer them the best I can, Musette and I are still new to the game but it is definitely been a sanity saver for us.

I am still dropping his dose, trying to get inside the enigma that is Samwise. LOL He's a bouncy boy and a confusing one. I am going to try and get some kind of curve this weekend. With the holidays, it's tough to find a slow time and be home for the curve. Sunday will be a good day.
