Dear All,

Big Kitty has been feeling much better today. He actually ate well and seemed quite cheerful. :-D What a relief for Momma...

I called the vet today to discuss tomorrow's dental and the drugs that will be given. The very good news was that she was NONDEFENSIVE and didn't get offended by my requests!!!!!!!!!!!!!!!!!!! Hurray- vets like this do exist, how great is that!

The not so good news is that she was going to use Metacam. I asked her not to and she agreed to use HYDROMORPHONE (a type of opioid) instead. She said they don't use Buprenex, but said this drug is chemically very similar and "less expensive." She said the side effect will be that BK is going to be drowsy for longer than he would be on Metacam. She also agreed to give oral ABs vs another injection, if he needs them.

So, overall sounds like a success- yes?

Anybody heard off/had experience with Hydromorhone?

Never heard of it. Good for you sticking to your guns and great to hear there are awesome vets out there that don't get defensive. I'm 0-2. Good luck tomorrow!!!!

Shadoe had the following according to the list on her charges:
Dental nerve block
1hydromorphone pain injection
dental IV fluids and anesthesia routine Prophy
1 Baytril injection (50mg/ml)\
1 metacam injectable 5mg/ml .... yes, I freaked on them because I had told them long ago to mark her file to say NO METACAM!

I don't think they will ever do THAT again, after my freakout on them.
They gave me codeine transdermal gel for take home but Shadoe HATES that stuff for some reason so now I tell them to give me bupe to take home.

I know how discouraging this can be...this is my vet #4 and I've had a fair share of frustrations with the previous 3. If you have any more choices, keep trying new vets until you find a "workable" one...

I'm still on a tryout period with this vet. So far, so good, but I don't want to get my hopes to far up.

Is Bupe an injection, or a pill, or fluid...?

I haven't used it myself, but others will tell you ....he might need the pain meds after though...if he has an infection.
I wish you well tomorrow....and hope that Big Kitty does really well for himself!

That is really good news about your conversation with the vet! :thumbup So far, sounds great!!

Thank you all for good vines for Big Kitty!

I am packed and ready. I'm taking his insulin, his favorite food (the vet said she will try to feed him when safe), and a blanket. I don't know if they will use it, but I read that anasthesia can cause hypothermia, and I don't want my big boy to be freezing out there before I come get him.

Are you supposed to fast him tonight? Depending on how many hours he has to fast, some people give a partial dose of insulin for PMPS the day before a dental. Did the vet give feeding/shooting instructions for morning?

Yes- fasting tonight from midnight and no water either.

Tomorrow AM no food/no water/no insulin.

I gave him a full dose tonight, cause I can still feed him through midnight, he was at 290, and he typically does not drop much after his shots. I wouldn't expect him to go lower than 200 really.

good deal. Those are the instructions I probably would have suggested.

The other thing we usually suggest is to test as soon as you can after you get home. For some cats the anesthesia can cause their BG to drop, even after they are home or throughout the night. Getting a test when you get home, then at PMPS, will give you an idea whether he is dropping from the anesthesia aftereffects. If so, then you'll want to be very conservative with his dose tomorrow evening too. Some cats get a lot of effect from anesthesia, and some get none at all, but it's something to be aware of.

Oh wow- I did not know that. Will be sure to monitor him closely tomorrow. Was hoping to spare Big Kitty some pokes after such a stressful day at the hospital, but I will test dilligently knowing that he can still drop after I bring him home...

I have not visited your condo before. But I thank you for educating yourself and discussing pain meds with the vet prior to the dental.

I wish Big Kitty (and you!) a stress-less day and quick recovery.

Bupe is given in a food syringe and shot into the cheek(no needle)--That is how it is absorbed--It is quite good, sorry you cant get it--But I'm sure they will give you something close to it.
Moonie's 1st dental she dropped when we came home from the dental, as the anesthesia ate up the bg's & she went low--She was fine & I monitored her a lot.
I'm sure Big Kitty will be just fine--Dentals are scary, but give Great results!
You'll see how good Big Kitty will feel--Paws & fingers crossed for an easy, quick dental!! Hugs!
