Yesterday

Increased to .5u this morning. He's been getting worse as the day has gone on. Under the bed/laying in the corner on the hard floor all day. Barely moving. Coat and expression look ragged. All the exact same behaviors he had when he was stuck in high BG back in May. I'm worried his BG is going to spiral out of control again if I don't do something.

@Wendy&Neko if you have a minute, could you please take a look at his spreadsheet back at his first p-itis event on 5/29 and how he very quickly climbed up in BG/high dosing requirements, and let me know what you think I should do? Does this look like the start of insulin resistance/toxicity to you? Given his history, I'm wondering if there's any way I can do a bigger increase for the next one to get ahead of it? Or faster?

On a lighter note, His microbiome results came back and they're also very bad (report attached in case anyone is interested). Looks like the antibiotic shot he got in early october wiped out everything and allowed some harmful strains to flourish. The fact that he's been on an incomplete diet of just boiled chicken certainly hasn't helped. But every time I've tried to introduce a new food or supplement he's ended up in the ER. I have no idea what to feed him anymore. He can't have carbs, fat, fiber, phosphorus, sodium--and those are just the things I know of. He was doing okay on his chicken & completer but they changed the formula without warning, which is what started this entire descent into gastrointestinal hell.

Is Charlie still getting Visbiome? Have you ever given him S. boulardii? It's a probiotic fungus that can mop up bad bacteria. I wonder if his tummy flora being off has been the cause of his recent numbers.

I hadn't realized TC Feline formula had changed. :banghead:. He can have a few carbs, why not fiber?

As far a p'titis start, do you have a stock of pain meds and nausea meds at home? I'd start him on that ASAP. This doesn't look like sudden onset of glucose toxicity or other resistance. P'titis/inflammation can cause insulin resistance, as can a bunch of other conditions. So far it looks like "a little off" compared to 5/29. With his current nadirs, stick with the 0.25 unit increases, if needed. That yellow could be NDW.

I also wonder if the gut flora is causing the numbers. I stopped the visbiome as of today because the top strain in it is one of the ones that’s taken over his gut 🤦🏼‍♀️I got a probiotic blend and fecal transplant pills from animalbiome at my IM vet’s recommendation. I believe the probiotic does have the s. Boulardii in it because it says it can help “outcompete” the strains that are too high in numbers.

TC added l-carnitine and some other vitamins and changed the way the krill is processed and where it’s from. I think it was the l-carnitine that screwed him up. Such a bummer because he seemed to be doing well on it. The IM vet had us try a couple different fibers for his constipation and they’ve both triggered severe gas episodes that landed us in the ER.

I do have all the p-itis meds stocked up and he’s been on them 😭 he gets max dose of gaba and Cerenia daily already. Bupe helped the night of the flare’s onset but caused side effects with the second dose. I had stopped the ondansetron but maybe I should give it for another day or two? The IM vet and I both thought this was a fiber-induced GI flare at first (it happened right after a meal with the new fiber in it) but now I’m not so sure.

Re: the inflammation—is there anything that you know of that helps with it that he’s not already on? He gets daily Cerenia and oral b12.

Oh, Jess, I'm so sorry. ☹️ I don't have any answers for you, but I hate that you're both going through this. I hope others are able to give you some advice. 💙🫂

I saw Xander is having some gut problems of his own. I’ll let you know how the animalbiome stuff works out. The gut restore (fecal transplant) sounds crazy but I know fecal transplants work wonders for humans with IBD or GI issues. So we’ll see

I use 1/2 capsule of S. Boulardii 2x daily for Ivy since she tends to have the opposite problem, she gets loose stools. That’s what starred with her when she had p-tiris.
It seems to be helpful for both issues.
I hope the fecal pills help too. Let us know what you think about using them.

I tried the fecal pills on my IBD kitty. I hope your luck is better than mine was. I have heard of at least one person who found it worked.

Thanks Staci 🫂 I just checked and it does have S Boulardii in it, along with some other things that are supposed to help replenish and increase diversity. His report says he’s missing sooooo much bacteria 😭

I’m glad it helps Ivy! Diarrhea is no fun either. I’ll report back about the fecal pills for sure 🫡

Ah, crap. Was it the same company? I’m hoping at the very least it will give him some bacteria back. He’s got next to nothin’ right now
 
The analysis results I got (same company, different cat gut) showed too high on some good bacteria, not enough on others. Way different than what you got.
