Since you suspect a possible infection I would take him to the vet. Ketones are possible with any number if there’s an infection and not eating and drinking enough. More common with high bg. His bg isn’t much higher than usual though. How can you tell he might have a fever?

I know it's not a sure thing, but his ears were really hot last nite. They are normal today.
 
I doubt it’s an infection. Has he had his mouth checked since the dental? I think. Would just watch today unless he won’t eat.

We are home all day so we can take him in if things take a turn.
I think his mouth is good, but I can have them look next time. He has great breath.
He is still eating, just not as much.
I will get a thermometer today.

I am getting alarmed at those climbing numbers, plus he didn't eat nearly as much as usual today. Lethargic, sleepy.
Definitely taking him in the morning, but any thoughts why this is happening?

I spoke to clinic and they advise no fast tonite. Bring him in after AMPS and food, then in afternoon they should be able to ultrasound for diagnosis of pancreatitus. Does that sound right to you?
 
An ultrasound might not even be needed. I would run the Specfpl blood test. It needs to b3 sent out and results come next day. There is an in house one but if in the gray zone will show as negative. Depending on how much they charge they could run the quick SNAP test and if positive it would be all you need.
Is this the first time she’s been inappetent?
 
I will defer to @tiffmaxee on this since is the expert on pancreatitis. I know you need the specfPL test for it.
The pancreas doesn’t always show up well on an ultrasound. If needed I would want it done by a board certified radiologist or boarded internist who dies a lot of them. They are only as good as whoever does it. I learned the hard way. Max had one by a vet who said he needed exploratory surgery. I went to my internist and he had the ultrasound repeated by his favorite radiologist who would not even look at the first disc and ran the Specfpl. Max never needed exploratory surgery. He had pancreatitis. He needed the ultrasound because he had been not right for a few weeks. That’s why I asked if this has happened before with Tansi.
 
This is the first time for Tansi.
The clinic where I take him has in house ultrasound and a full time radiologist. It is not a regular vet clinic and besides the emerg department, it is all specialists so he will be seen by internal medicine.
I will make note of all the blood tests and insist on them within reason, when he goes in.
I think the idea was to avoid fasting him overnight and then no insulin in the morning. So he will go in after am shot, and by the time he gets the ultrasound, he should be ready for that blood work also. Then home in time for food and pm shot.

Make sure you get an RX for ondansetron for nausea even if they give him a shot of cerenia. It works better for nausea.
