Good evening,

another day sky high. After I found this hard mass yesterday under her armpit/breast in combination with the licking and the high bg I became so nervous, that I went to the vet today.

I hoped the vet would say that she felt the hard mass before and that it probably is scar tissue from the surgery. But the lump was also new to her and she said it is in an area, which was not involved in the surgery. She said that cancer is a possiblility besides an auto immune disease, I forgot the name of, because I was so overwhelmed for the moment. She said that even though the pathology after the surgery said, that there was nothing cancerous, there could be something which was not in the tissue they tested.

She did a fine-needle biopsy but said it is not guaranteed, that she catched the right tissue. It can happen that the needle gets clogged by fat before it reaches the structures you want to investigate. So we have to wait, what the results are and then make our decision how to go on.

She gave Binie some procaine injections and for me it looks Binie is better after it. She didn't stop licking completely but not so hard and not so often anymore. And her appetite was good for dinner.

I showed the vet Binies insanely high bg and she doesn't think it has to with the antibiotics anymore. She thinks it is a sign of something else going on.

I was not prepared we have to deal with something new now and I find it very confusing. I try not to overthink it before the results come. She said they will most likely come during the week. When they come, I may ask questions here.

So not so great news from our side. I hope everyone else is well and had a good start in the week!

I'm sorry about all that. Could it be lymph nodes from her skin infection on her breast? I guess the results will tell.
For the BG, I know that Bella was not regulated yet, our vet had decided very early that "there must be something else, or maybe she is insulin resistant", but once I started coming here and increasing and testing, it appeared that she just needed the right dose. Hopefully you will find what that dose is for Binie as well.

Thank you so much Cecile! For lymph nodes it is too big and in a strange shape and place. We will see, if we get results from the biopsy. Anyhow we also have to wait for the urine test results and if we managed to erase this multi-resistant germ. Seems Binie has so many things going on. I just hope she is not too unwell.

I hope you are right about what her bg means. Maybe the vet was too fast, to draw conclusions from that. I will try to take it step by step now...

You are doing everything you possibly can to try to figure out what is going on with her so that you can do everything you can to help her.
We have to wait to see what the biopsy shows and hoping that she was accurate in getting a good biopsy.
Also hoping and praying that the antibiotic took care of the infection.
I know you must be crazy with worry, and I understand completely, I would be too.

Remember to breathe and focus on what you can take care of at the moment, which is her glucose and feeding her and keeping her comfortable until you know if anything else is going on. One thing at a time.

Try to get some rest if you can and do some of those exercises you sent me the other day.
We are praying for you and Binie to get some positive news this week.

Thank you so much Staci for you kind words! And you are so right. I should concentrate on what I can influence and I try my best to do so. I'm happy that Binie is much better tonight than yesterday. Seems like the procaine is really doing its job and I know, we can do it again if necessary. My main priority is to keep Binie comfortable. Just now she did a walk on the terrace, which is a good sign she is quite well.

We'll see what the biopsy says. Hopefully we get a result we can work with. And the week after I can hand in the urine for a test.

Do you think it is right what the vet said about the biopsy? That it might not work if by chance she is only getting the cells out which are before the tumor or whatever this lump is? Must be the same with humans, isn't it? I never heard anything like that.

Unfortunately yes—needle biopsies are like throwing a dart blindfolded. You hope you got it but you can miss things. That is the same with humans. We often biopsy things with needles, sometimes guided by Ultrasound, but there’s always the chance we can miss a cancer that way (so surgeons always warn patients and pathologists usually put all sorts of caveats in their assessment). Even if you hit the mass you are aiming for, it can be a mix of reactive tissue and tumor cells and you might only suck up reactive (scar) tissue.

In humans a common needle biopsy is breast mass. I think most people quote false negative (missed the cancer) between 2-4% but usually they use ultrasound to guide the biopsy

I think the lump is so big that missing it is not really a big risk. But not getting the right cells seem to be.

Yes surgeons would say the way to know is once it’s in the pan, meaning taken out during surgery and sent to pathology. That is the only 100% way to know but you’d have to weigh the risk of anesthesia/sedation with her age and condition. It’s not an easy decision.

My gut feeling says that we should absolutely not do that. But I will wait for the results and than think about it again and see what the options are. No need to make a decision now. Thank you so much for your expertise, it helped me to trust my vet!

Yes. But in a large mass there’s better chances of getting a good sample (because whatever is causing it is good sized, even if there’s reactive tissue surrounding it or mixed in). Fingers and paws crossed the sample is good, and miss Binie gets some good numbers soon
