10/11 Oberon PMPS 330/3.4, R trial 1.25 U, +1 330, +2 300, +4 328, dose increase tomorrow am

Coming back up at +5... looks like the low at +4 was down about 50 from AMPS. I'm thinking R could be bumped up to 1.25 next time.
 
Spent part of the morning trying to learn more about antibody responses to chronic antigen exposure (hoping to understand his periodic dips). Nothing so far, but I did learn that insulin antibody-antigen complexes themselves can build up and lead to problems like renal disease. Now I'm wondering if that's why his kidney vanished... Need to read more, though. This wasn't specifically about cats.
 
Could be... the ultrasound report said they couldn't rule out either a congenital problem or surgical removal, but said it seemed unlikely. So far his renal bloodwork is fine, but with only one kidney I'm going to keep a close eye on it. And it makes me that much more hopeful that his antibodies will give up sooner rather than later. (Higher insulin doses --> more antibody-antigen complex --> greater risk of renal problems?)
 
300 at +2; I'm probably going to give him a little break and just test at +4.

Thinking ahead to tomorrow... this is cycle 9 at 6.5 U of Lantus. So he should get an increase tomorrow evening, which would mean no R then. But tomorrow morning he has a tech appointment for bloodwork (rechecking PCV and CBC for anemia) at 9:30. I'd be able to do a +2 check, but definitely not +3 and maybe not +4, depending on how long it takes, so I should probably skip R in the morning. Given all that, I'm considering increasing the Lantus to 7.0 in the morning (a cycle early, but given the IAA I think that's justified), which would allow me to potentially give him R in the evening. Thoughts, @Wendy&Neko or other IAA experts?
 
Another thing to consider. Tomorrow night is cycle six since the greens. It's possible he could break the bounce either tomorrow night, or the morning after. You don't want to give R on a potential bounce breaking cycle.

I think you could increase the L in the morning (that 98 was blue if you squint right ;)).
 
Been doing some digging in the vault. :cool: Answering your question in Howie's post (new IAA kitty), I didn't want to comment there because they aren't using R. Take a look at post 11 and 13 here and here. TOR = touch of R. Sandy was the IAA guru around here - first kitty here diagnosed with it. More here and here from her. And one on IAA and Ruminations.

You asked yesterday about R mid cycle. It's something you could try once you've got a handle on a good R dose at preshot time. You could try it around +6, when he's significantly heading up again. I'd start with a smaller dose than you do at preshot - at least until you learn more. +5 a couple nights ago would have been an example by +6 probably, unfortunately being at night you couldn't monitor. But worth trying if you see something like that during the day when you can monitor. If you do a mid cycle try, don't do the following preshot R, so you can see more of what the effect. Cool, thanks! I'll read them all asap. (I tried searching the forum a bit earlier, but it turns out that "IAA" is too short a string for the search engine, so I have a feeling I'm missing stuff.)

328 at +4... I'll get one more check at bedtime.
