If you have good results on cycle 1 of a dose, with a U-curve and a dip to a better-than-normal nadir, but then lose those results on cycle 2, my understanding & experience with PZI is that it means their body has overcome that dose. The cycle 1 result is the "wow factor", and then after they get over the "wow", they go "nah, not enough insulin". Someone (Rob?) described it once as like a drug addict - keep needing more to get the same insulin high. Of course other explanations are possible I suppose, and you don't know for certain what happened last night - for instance if he got a good nadir, today could be a pink reaction cycle. Given his history though (no known low nadirs, and signs of possible insulin resistance (?)), I would suspect there was no good nadir last night.
I would probably give it another cycle tonight on this dose, and if you can, get a nadir test. If no improvement, I'd consider raising again. I'd like to hear what others think though. Especially as you get into higher doses with not a lot of response, I start to feel out of my league. Have you ever tested him for insulin resistence? I don't think the doses are high enough to suspect Acro, but if you haven't looked before at the symptom list, might be worth reading through and seeing if it rings a bell?
