1/4 buprenorphine question

i would like to know if the dose Rx to Rocket is ok to give....

buprenorphine 300mcg/ml (no flavour) oral solution
give 0.52 ml under tongue three times daily for 7 days

he weighs 7.13 kgs as of today

HELP...need to give shortly and would like to know if the dose he has been given is ok to give.....

sorry i had to use 911 icon but with all the other meds he is on i need to give this shortly as i need to space it out and time it with the other meds

i only managed to give him 0.32 of the Rx dose...

but i still have questions if the dose that has been rx to him is good...

also is it meant to be given on its own or mixed in with some sort of liquid?
i ask cause he takes Calcitriol and with it i add to the syringe some Clinicare....

just wondering on the buprenex

Concentration is 300 mcg/ml
Dosage is .52 ml
Therefore you are giving 156 mcg.

Weight is 7.13 kg
Therefore you are giving 21.88 mcg per kg.

A dose of 20 mcg/kg should be appropriate for 8 hours of pain relief.
What your vet has prescribed sounds appropriate to me.

In my own experience with cats who are quite sensitive to drugs, and are on a few meds, I prefer to give a bit less. I might start by giving this "8 hour" dose above once every 12 hours if you find that it is very sedating for him.

It is meant to be given on its own. You can give it under the tongue, or it may be easier to squirt it into the cheek pouch. It is meant to be absorbed through the membrane, not swallowed orally. (If he does swallow some, it is not harmful).

i got scared with the swallowing part as you mention....i'm certain it was under the tongue....Calcitriol is now in 2 1/2 hrs....poor thing...will update Lantus board when i can get everything here sorted out at home...

cleo got 0.24ml of that concentration twice a day but she was around 4kg.
his dose is every 8 hours, which is good. that's about how long the bupe lasts in system, i think.

no need to mix with anything but don't put anything unsterile into that container. use a sterile syringe every time. try to get it between cheek and gum. under tongue is optimal because the veins under there carry it best, but i know it's hard to get stuff under their tongues if they're fighting the med.

thank you....will keep your input in mind....

now is it normal for them to be lip smacking....i went to peek at Rocket and see how he was doing...as he is on his bed in the bedroom....he sort of looked at me and lip smacked a few times....is that ok?

he takes zofran too....but that's not due for a few hrs yet....he also takes zantac....oh my poor kitty on so many meds....because he was taken to the vet he didn't get the zofran this morning....

And you don't have to get it in the mouth all at once.

Put in a little bit, a minute or so later, a little more...etc.

i started it at 3:15 and finished at 3:30...and did not manage to get it all....hence the reduced dose....thanks for the info...

so now i guess i need to use a different syringe than the one previously used to draw the liquid, right?
sorry if i ask stupid questions but my brain is fried and can't think straight and honestly i rather ask a dumb question than not ask it at all and be second guessing my every move....with little sleep...anything can happen

Well, I never did that.

I used a 1cc TB syringe with a 25ga needle. Draw up the dose from the vial.
Then CAP and REMOVE the needle, and use the tip of the syringe like an oral syringe to
give the med.

I used a new NEEDLE each time I changed to a new 1ml vial of Bupe.

You can also give the pain reliever Sub-Q...you would need to ask your vet about
that. You would need a new needle each time for that.

sterility is less of an issue with oral bupe than with injected bupe but yes if the syringe has touched his mouth it shouldn't go back into the bottle unless it is sterilized, or you risk contaminating what's in the bottle. so i always used new oral syringes to draw up and give the bupe. at one point i had bupe in an eyedropper bottle and could give the drops without the dropper's touching cleo's mouth at all.

from what i've read you could sterilize an oral syringe/dropper by putting a little bleach in a glass of water and drawing that up and shooting out fully into sink once, followed by 3 syringefuls of plain water, but it's better to use a fresh oral syringe every time if you have them.

lip smacking is probably a sign of nausea. you could give the zofran a little early. it really stops the vomiting/gag reflex. it's once a day but you could be a little loose with the actual time in the day.

One mcg is equivalent to .001 mg.

The range above translates to 10 mcg - 30 mcg per kg

Rocket weighs (approximately) 7 kg, so his dose range, according to the above, would be 70 mcg - 210 mcg (.07 mg to .21 mg)

As your Buprenorphine concentration is 300 mcg/ml, your dose range would be .23 ml to .70 ml.

The prescribed dose was .52 ml (156 mcg or .156 mg). The dose you actually gave was .32 ml (96 mcg or .096 mg). Both are within the range quoted above. You are fine.

as of this morning i gave 0.35
and i'm ok with that....he was spaced out yesterday.....but he did eat on his own late last night....i'm hoping to stick to the 0.35
 
just wanting to know if the bupre makes them not hungry...
I have had to syringe feed Rocket today...did not give full dose this morning only 0.35 as it is too much going on in the household...this afternoon he will get the full 0.52 dose Rx hoping that the peace and quiet also will help him...

he is not eating on his own....even with such low dose given...

thoughts on that?

If Bear Man was too sedated, he just preferred to sleep rather than get up and eat or use the litter box. My civie is on bupe for arthritis pain, and her appetite has improved a bit as her pain went down (I started her on other meds which could also be helping). One bad side effect of Bupe is that it is constipating.
