TL;DR Pain meds helping, eating more. Likely switching from Lantus to ProZinc tonight or tomorrow depending on delivery timing.

Question: Is there anything specific to keep in mind when changing insulin types?



We did get gabapentin last night and it helped him eat a full meal over about 4 hours, yayyy!! :DIt does look like he's grimacing less while it's active. I did go ahead and start his Atopica again without waiting for response from our out-of-office internist, as my vet and I were heavily leaning that direction. Based on his blood work history, I would not be comfortable with him at such a low dose of prednisolone without it.

Pants was low to mid 200s in blood glucose, so I did give insulin last night. Kept it sparing at 1.5 units. With 134 this morning, even though that's higher than I'd like, I'm inclined to just monitor him -- I don't know how the gabapentin and Atopica will factor in with both freshly added

Our ProZinc prescription and needles should arrive tonight for us to officially switching insulin. I did find some scabs near some of his injection sites, so I think this is the right move.
 
Biggest thing to consider when switching insulins is the dose. Note, we very seldom get people going from Lantus to Prozinc, but typically start cats going from Prozinc to Lantus at about the same dose, depending on how kitty is doing at that dose. Keep in mind that any Lantus depot could be still in play. I can't tell from the spreadsheet whether you shot insulin 28PM or 29AM. Please put NS in the Units if you did not. Whether or not you shot insulin would help figure out how much the depot would be in play and whether you should be a bit conservative on dose to start.

Sounds like you have the U-40 syringes for Prozinc. Good luck.

Thank you! The insulin arrived this evening, but the U40 syringes did not arrive with it... 😑 Apparently that will be a separate package tomorrow. I got home too late to get any U40 from the pharmacy. His glucose was under 200 around shot time, and based on his current state, I held off (still need to update bg on spreadsheet, but I made sure shot dosing was filled properly now 🙏)

When I got home, Pants greeted me at the door like he used to before the mouth pain, and went to crunching on dry food. He's like... back to his normal self in spite of the hole in his gum?? I'm baffled. Other than one half dose 1/29, he has not had Lantus in several days... And he had a full, solid stool this afternoon. No vomiting or diarrhea. I'm even more convinced now that switching to ProZinc is the right move
