I'm a little out of touch due to internet outages, and life happening stuff.

Bear update:

I have changed his pred dosing from 15 mg at night to 7.5 mg bid. His numbers are still not great. I have to add some fat onto his .5U, but I would rather wait for a day off to do it.

He had his bloodwork recheck yesterday, one month after his Tapazole overdose.
Good news: T4 in normal range, and WBC normal. Blood glucose normal :smile:
Same old news: Still anemic, potassium still at bottom of normal range.
Bad news: renal values are still creeping up. Urea expressed in US units is 50.42 (18.0 mmol/L), and creatinine in US units is 2.52 (223 umol/L).
New news: Globulin just above normal range.

O/T Teddy update and question:

He isn't doing so well (no appy, liquipoo, very bad hematochezia). I am taking him to see the vet next week. This could be worsening of his lymphoma or some other issue (vet was suspicious of pancreatic cancer recently). I wonder if it could be an IBD flare, and want to try some last ditch ideas before moving on to re-imaging and more aggressive chemo. He is currently on Leukeran and sub Q dexamethasone (upgraded from Pred after two bad CP flares). This already puts him at high risk for diabetes. Here is my question: has anyone ever heard of Budesonide and another steroid being used at the same time? I am wondering if the Budesonide could help the IBD with some local action, and the Dex can keep the pancreas happy. Is it too much steroid? I have asked the vet about it, and she says it could be done. I am also going to start him on a course of Tylocin for the diarrhea. In case anyone asks, yes he eats crappus food. :oops:

I plan to discuss treatment options for Quincy's lymphoma/IBD at his next vet appt (2/16). I'll try to see what he thinks about using different steroids together or other things to supplement the leukeran. Maybe that'll be of some help to you, although it'll be a few weeks in coming.

In the meantime, sending good thoughts your way, Linda.
 
linda, i'd probably PM jess, larry, or jojo with that question (or any other vet tech on board).
harry's had 2 different steroids but never at the same time. started on one for a week, then switched, for two different problems.

Sorry I can't help with the medication issues, but glad that some of Bear's bloodwork is good news, I hope you and your boys have a good night tonight.

Thanks Chris. I did pm Jojo on FB. She hasn't seen it done before. I also posted on the non diabetic health forum.

((((Teddy)))) I have been curious about this same thing recently. My vettybean should be calling tonight with Sasha's blood results....I can have her poke around VIN and other vets if you want.

I would think that if you are monitoring the liver, kidneys, and CBC often enough, you would catch any system that it could potentially impact. Budesonide is processed mainly through the liver and can hit it hard, so if there is any liver abnormalities it should probably be avoided.

I've sent a PM to the 'roid Queen (Cheryl and Winnie) asking her if she's come across the answer in her readings. Maybe she'll stop by tonight.

Incidentally, I was checking Bear's meds for interactions (just being nosey!) on this page today, and I came across this at the bottom - doesn't address budesonide, but it talks about using pred and dex together, with pred dose being reduced to account for the potency of dex. Scroll down to Therapeutic options for inflammatory bowel disease, see IV.B.3. Dexamethasone.

in case you didn't see it, jess answered and said don't do it. risk of GI perforation is too great.
(we suspect cleo may have a touch of anemia from pred alone so a 2nd steroid would probably be too strong)

since i talked to you quite a bit today in FB PM land and i'm sick as dog, just stopping by here cause i wanted to see SS. oh geez linda it is no where near as bad as i thought it was going to be from your description. a little hair plucked off jill's nearly bald DBF should be all ya need to counteract the rise in range you have going on. no rush, when you are off next.

i'm not done with the FB conversation yet, i left you some questions to answer about teddy's fuds.

ps. i didn't say at the time, but was thinking later when curled up in bed and didn't want to turn computer back on that the double steroids thins for a cat with chronic kidney infection and the suppressing of immune system that steroids produce probably wasn't going to be a good idea.
