1/19 Latte - Dose Decrease Advice

I would like to know what is recommended when needing to decrease insulin for a cat not eating and unpredictably vomiting. Her vet, in the past has said to cut in half or skip shot. That seems like a big jump down and tremendous set back if she decides to rally. But she needs to be safe.

hmm, my brain doesn't think very well in absolutes, especially when you're talking about permanent reductions. If I felt like I had to make a permanent reduction, I would probably reduce by 0.25 at a time until I found a dose that was comfortable for me. Others might have different advice.

I guess I'd be like Libby, take the dose down .25 and call it a semi-permanent reduction? If she isn't eating, just taking the 'fat' off the dose might not be safe enough. Is that what you were thinking?

Well, ya - semi permanent until, or if she can become more consistent again. Sorry if I was not clear. Obviously nothing is permanent in this game. I hate to see her running higher, but when I saw the 79 at +2 tonight, and her in a meatloaf position not more than 30 min earlier...well...crap! was all I could think of. Of course I know she may go up from pred and/or bounce from 2 big drops today. What if she had vomited at that point?! What if I was not home?

The only time she eats right now is when she drops big. She drops big because she has no food on board. Im guessing that cant be helping her feel any better either.

Am I wrong for thinking a decrease might be a good idea? Is it a bad idea? I dont know. I really dont. I dont want to ruin progress, but I dont know if you are suppose to give so much insulin to a cat who is not eating/vomiting. Kind of catching the vibe it might be frowned upon. Have others not done this before?

sorry, I appreciate the suggestions...I was not sure if there was a protocol for dose reduction outside an 'earned' one. Im really sad to think about needing to do this because if it were not for the lack of eating/vomiting, she is almost exactly where we have worked so hard to get her...with maybe even more potential.
 
I really wish I had something wise or insightful to offer. Any chance the potassium is contributing to this? It looks like some of the side effects include stomach pain and vomiting. It might be something to discuss with your vet.
 
((Carolyn)) You are not wrong to be thinking anything! You have been working non-stop
to help Latte and you know what happens when she doesn't eat with the insulin onboard. I think it
is a very logical question.
You are right there and can see what she is doing and how she is behaving....each day can bring different things...
so maybe you have to take it one step at a time....one shot at a time with decisions about each new day....

I don't think anyone here would frown upon it at all. You have to do what works for her and you. I agree with Libby and Carolyn about decreasing the dose overall and not just BCS on days that she isn't eating well, simply because even with a BCS dose you will still have previous shed action working.

My only worry is that I know how much the high numbers bother you too, and it is a very difficult balance. That would be my only reason to suggest inching the dose down slowly and giving it a few days at each level until you found the right dose rather than taking a full 0.25u off the top.
