Here is yesterday for Eddie:

Starting off in the yellow zone today, so hopefully we'll even out.

Tomorrow is the day I finally speak with the radiation oncologist about decreasing Eddie's Prednisolone dose.

He's been on 2.5mg BiD and it's possible we may get it down to half that. I hope so for as we know Prednisolone is a commonly used drug in cats and "potential adverse effects include hyperglycaemia and diabetes mellitus" (https://journals.sagepub.com/doi/full/10.1177/1098612X20943522 ).

So, Prednisolone coupled with SRT, which ought to be adversely affecting the tumour--meaning a reduction of the need for insulin--might be seen to create a push-pull situation for Eddie wherein the Pred is fighting with the SRT...

Looks like Eddie kept on dropping down last night, as his trend was showing yesterday.

What was Eddie's pred dose before the vet upped it after his latest SRT? How much pred affects cats is ECID (of course). 5 mg a day is not a large dose for a cat his size, but it's possible you could see a couple units drop with less pred in the picture. I'm hoping it might flatten him out a bit too, as the pred effect doesn't last all cycle, possible contributing to some or the uneven numbers. Of course, there is that ECID factor. My current kitty on pred for years, much smaller than Eddie, has practically no change in BG from it.

The SRT effects easily out fight the impact of pred.

Prior to SRT Eddie was on .625mg/once daily (1/4 2.5mg tablet) . He's now been on 2.5mg/2x daily for 109 days and before that, immediately post SRT he was on 5mg/2x daily for two weeks), so compared with his previoius (pre-SRT) dose, it's pretty high. The IM vet here suggests Eddie might go down to 1.25mg/2x daily so we'll see what the radiation oncologist says.

As for the SRT effect vs Pred, I'm thinking that the Pred is steady-state while the SRT has intervals of GH secretion as the tumour "fights" back, shall we say. Therefore, on a daily basis, the constant has to do with whatever the ratio is between the Pred effects and the fluctuations of SRT/GH.

We'll see what tomorrow brings. If anything, we'll be reducing but I just don't know to what. I don't imagine they will cut the dose in half immediately, but who knows... Of course, it will all mean keeping a closer eye on Eddie...
 
What was the reason for going to twice a day pred? If tapering, might be easier to just go from twice a day to once a day. One less pilling for you.

It was only for consistency with his other medications: he gets DDAVP (0.5mg/2x daily) and Levothyroxine (1.0mg/2x daily). It also seemed to be about not loading him up with Pred in one go...
