Good Morning,

Whiskers had a high AMPS 407 today Red. I don't know if that was from the stress of the veterinary hospital yesterday. Last night PMPS was yellow. Go figure.

The IM vet left a message on my phone this morning, say he heard I had more questions and will call back later, or I can call him.
So when I talk to him I'll let you know.

407 is almost pink!!!

I spoke to the IM Vet this afternoon, he said the results from the cortisone test has come back, and as expected she is producing to much cortisone. The IM vet recommends a medication called Trilostane 10mg w/food BID, to better manage cushings disease, this may lower her insulins needs. It should help with other signs of cushings disease as well, thin skin, bald patches (she had a benign cyst removed from one of her rear hocks this fall, and fur hasn't grown back), rounded pot bellylike appearance (I just thought her tummy was a little chubby), excessive thirst (maybe help with IE). In 7-10 days we will have to do an ACTH stimulation blood test to see if the cortisone production has been reduced. It is similar to the test done on Friday, but it only take an hour as opposed to all day on Friday.
After one month if her immune system has improved, we could consider surgical removal of the malignant adrenal tumor, this is risky as diabetic/cushings cats are fragile, prone to shock, bleeding.
The IM vet also said she has two somewhat enlarged lymph nodes, it could be caused by her conditions or not. (my mom is a retired RN, she pointed out it could be due to infection, we are continuing treatment w/mometomax of her ear infection).
I spoke to our regular vet as well, he said 10% of animals can die from the treatment with trilostane.....he said he didn't want me to become upset if something happened. I told him I was already upset, so my state of mind in my opinion is not relevant. He suggested we treat the cushings on Friday, so it was a little unnerving today. That said he told me if she can take pills....pilling has not be a problem with Whiskers. He would go ahead and recommend treatment.
I think that is most everything. I'm supposed to pick up the medication tomorrow. Does anyone have experiance with Trilostane?

Hi Cheryl - I am glad that you were able to connect with all vets concerned. I can't offer an opinion - I have no experience with the drug. I wish you guys the best - and comfort with the plan of treatment you choose.

Glad that you got confirmation of what is going on, even if it isn't great news... it sounds like you have a plan of action and everyone is on board, even if reluctantly. Lots of healing vines coming your way! You guys remain in my thoughts and prayers.

I want to refer you to a website that has a comprehensive discussion of Cushings. What I'm curious about is that from this site, Triolostane appears to be used primarily for pituitary-type Cushings. Lysodren, while also used for pituitary Cushings is an option for adrenal-type Cushings. At the very least, can the IM vet discuss the different medical therapy as well as surgical options?

I'd also suggest asking about this on Health.

i agree with Sienne in asking the different treatment options with the IM vet....

i did a lot of research on the drugs back in 2008 as i mentioned to you via pm...but it was not for Rocket but rather for Comet (the dog)....and the preferred drug was Lysodren in every forum i signed up for back then in the hopes of helping Comet and vet reports i read...so i know Whiskers is a cat but the info i sent you via pm also talked about cats not just dogs....

do ask the vet lots of questions...

Hi Everyone,

I'm really overwhelmed. I just don't understand why Whiskers is so sick. It's just not right, and it is just so hard. I've spent the last our trying to join a group about cushings disease and can't figure that out, so now I'm really frustrated. I have tears running down my face. I'm gonna take Whiskers and the rest of the pets upstairs and lay down. It's pretty late.

Sienna, thank you so much for the link today and the one the other day. I'll take a look.

I'm just really discouraged right now. Sometimes it is just to much.

I just went to the link that Sienne sent. Whiskers does have adrenal cushings, but I don't see what medications are used to treat that. I did see the area indicating that Trilostane is used for pituitary cushings, but not what is used to treat adrenal. I do remeber the IM vet talking about the fact that Whiskers did not have pituitary cushings. I'll call him tomorrow and discuss why the choice of Trilostane? I really feel out of my element here, and so over my head, when trying to sort this out nailbite_smile .

Get the information from the vet. Take notes or even tape the conversation so you can go back and review what's been said. You need time to digest the information and get past that overwhelmed feeling. It's no different than when you found out that Whiskers was diabetic. No doubt you were overwhelmed. And then you did your homework and found this place.

There are several vet techs who are around on Health. Use them as resources. They can help you understand the treatment options. Remember to ask the vet for the pros and cons of each of the options. Also, just so you are aware, if you want an outside, independent consultation, Cornell University Vet School will do phone consults.
