01/01/201 Amps 8.8, hold dose?

Hi Charlene, unable to access your SS. You need to share it for anyone to be able to see it.

Have you fed Sir Thomas? If not, can you please test him 20 mins after the last test and see where he's at?
Will you be able to monitor him if you give him a shot this morning?

Btw, your Signature doesn't say which dosing protocol you follow - TR or SLGS.

Oh whoops! I ran out of storage and had to make a new one and must have forgot..it is public now. We follow SLGS and updating signature now, getting reading now :) we will be able to monitor him today

He is 9.5 now and eating again too
 
Can see your SS now. You will need to add tabs/pages for 2022. I can help you with that.

I see you've held the 4.5U dose for over 3 weeks now. With SLGS you do a curve once a week and evaluate the dose. You also want to get a few spot checks whenever you can. Preshots do not give you a complete idea of how your cat is doing on a dose.

Done! Thank you so much! We have held off changing the dose as the Internal med specialist vet was considering switching insulin type instead and I've been nervous to take things into my own hands but I will start. And I can get a couple spot checks or curves on weekends so I'll do that. It's been hard with his IBD and he has an unknown liver issue that he needs a biopsy for, we are just saving the money for it.

We can help you with that if you follow either method we use. Which insulin was you IM suggesting? The only one I would consider is Levemir.

We have an appt coming up in a couple weeks to discuss the insulin change, he didn't say what kind but I will ask about levemir. I am nervous to switch because other than the big pees, he is doing great for once. Purring, gaining weight, playing. He figures it is his liver issue making things difficult, as we have tried medication to change his liver numbers to no avail so we are not sure without the biopsy. We have been containing to try to find wet food he will eat, but not having much luck yet so he stays on kibble for now as treating the IBD has been our first order of business since in the past when we try to switch foods, he stops eating and ends up in ER with fatty liver, pancreatitis, key tones.
 
It took Max who had pancreatitis about a year to become regulated. I stuck with lantus because I knew how it worked with him and the possible later nadir would not have worked with my schedule at the time.

That's hopeful! Should I worry about how high he needs to go? 4.5 just seems so high, but he is on kibble still...
 
Do you test for ketones? With high numbers it’s a good idea. Are you able to do a curve to see how low his dose takes him? Lantus is dosed based upon the nadir, how low it takes your cat. That is generally around +5-7 but can be earlier or later. If he’s only reaching the 200’s and no lower he needs an increase. Glucose toxicity probably is causing him to remain high due to holding the dose too long. We don’t know because you don’t have any mid cycle tests. have any tests Can you test every 2 hours for 12 hours tomorrow starting with amps or every 3 hours for 18 hours? Is it possible to do that weekly?
 
I can definitely do that this weekend and most weekends. Sometimes I work the odd weekend and can't but I can start doing as many as I can and will test him every 2 hours today and 3 hours tomorrow.
 
I just noticed on 12/8 he dropped under 90 so you should have reduced. Now that I see the spreadsheet and some blues and greens.:) The black pmps was likely a bounce and he probably went lower than he was used to that day or the day before. Those extra tests really are needed.
