07/13 Jackson +12 79, AMPS at +12.5 82, +1.5 110, +3 100,+4 101, +5 96, +6 91

Looking back at his spreadsheet, the only time I shot a green was June 4th after his cardiologist appointment and he had the 70ish numbers in the AM cycle and I shot an 83 at PMPS and reduced his dose by 0.5 unit.
I guess I'll try that again.
Testing again now for a +12.5.

At first I took his latest and greatest regular dose of 14 units and mulitplied that by 0.75 to get 10.5 as a 3/4 dose (or a BCS dose).

Then I decided especially with him rising slightly, to give him the decrease from 14 units to 13.5 units as planned this morning. It may have been closer to 13.25 but that's a little irrevalent at this dose I think.

I don't want to stop his momentum, I work from home and can test several times a day, and I have some HC that I can mix with his LC to make MC if needed.

I did wake up to the two alarms (phone and plugged in) twice last night, so they do both work, when I wake up to them. I don't know why the phone shuts off it's alarm after a while of no response from me. Last night I did not incorporate the sould of my alarm into my dreams.

I did get the Lantus for $35 yesterday.
I asked the lady if that will be $35 and she said that will be $90. I asked if she used the GoodRx and she said yes. I said I paid $70 last time, and she said the GoodRx flucuates the discount. I shook my head. Then she said she ran it through some other system and got it to the $35, so that's a win. Hopefully it will be the same next time.

So glad the coupon and pharmacy figured out how to get the $35 price.
I always remind them I don’t have insurance and need them to use the coupon (which I go in armed with a screen shot they can use).
Just need a pharmacist who knows how to apply the coupons.
And willingness to help and think. 🤔

I’m curious what the suggestions are, given his use of cabergoline and how it affects the dose and trends.
 
Sorry I wasn't up yet when you asked your questions. To answer the question of what happens when you see a green PS. With SLGS, you don't shoot if under 90 and I think you were still following TR when you last shot a green preshot. It looks like a lovely flattish cycle today, so I hope all will be fine.

With possible cabergoline action starting, instead of using 3/4 of the dose for a BCS, I'd go to a 1/2 dose, especially with that largish depot. If you had skipped, that would have been OK too. Note for anyone not Dyana, this is special suggestions for her and Jackson since he has acromegaly and getting cabergoline which can make rather sudden needs for less insulin. Another thing you can do is to reduce by a full unit at this size of dose. For people over 10 units, changes are typically 1.0 units at a time, unless tweaking a dose when increasing.

As above, I've seen some really strong responses to cabergoline. Safety first. It is much easier to go back up if you need to.

I thought I switched to SLGS because I set his reduction number to 90.

I did not want to set his reduction number to >50 for a cat on such a large dose. At the time he was not on Cabergoline, and I saw no reason to hope for remission.
I do not have experience with SLGS, but tried to read up on it this morning.

I had some intuition, and was wondering what to do if he was under 100 when I woke up, and he was.

I had "do not copy dosing" on my signature. I followed mostly TR, with some mods for post acro treatment safety. Wouldn't hurt to add custom dosing in case lurkers are about.

Whether or not remission is a possibility, a lower dose is likely in the cards.
