budesonide and remission

Hi all-

Long story short, has anyone started a kitty on Budesonide while kitty is currently in remission? I know each cat is different, but I worry about developing diabetes again. He became diabetic from pred and tooth issues the first time around.

Thank you so much for any experience you can share.
 
Sean, I know Jayla is using budesonide and it has raised the BG requiring tiny amounts of insulin from time to time. Some do well on a small amount of budesonide like her cat and hover between normal and out of range. In my opinion using it is better than not as chronic inflammation can lead to lymphoma if allowed for long periods of time. I’m pretty sure that is what happened with my civvie who went to the bridge when only 12. By the time we investigated she had intermediate cell lymphoma. JMHO. :bighug:

Thanks for the info! It is so strange, he used to vomit multiple times a week, starting about 8 years ago, and now rarely does. But he has a true allergy to turkey I discovered. Even when he was vomiting all the time his appetite was there. Then about a year ago he stopped eating, and all of a sudden lot s of issues. I'm not sure if its the stage 2 ckd, or teeth issues (again), or the inflkammation causing everything. Could be all 3. He went under for dental in Feb, and now might have to do anesthia 2 more times. I don't lime that at all. Our goal was to get him to eat a novel diet and see if he improved, but he is so so picky now and only eats friskies oceanfish , which might have turkey bits in it for all I know (I think the can says poultry). Anywats, I would hate to put him on bude and develop diabtes again if its not even needed. But I guess at this point it either start the bude blind or do a biopsy :(
 
Noah has lymphoma and developed diabetes either due to the pred or lymphoma vulnerabilities or both? My all-cat vet changed him from pred to budesonide in an effort to see if he would achieve remission from the DM. Unfortunately it didn't work but that was the intent.

Oh that is too bad. How long has he been on Bude?
 
It's hard to decide what to do sometimes. When my kitty was actively diabetic ( he is in remission now with no signs of diabetes) he was diagnosed through endoscopy with IBD. I was really conflicted about giving him the budesonide for fear of the diabetes returning. His specialist felt it was more important to get the intestinal inflammation down as it can cause a lot of serious problems down the line, and diabetes (if it did recur) from the budesonide could be dealt with insulin. It turned out though that his IBD has gotten much better on his current diet and he has stayed in remission from the diabetes. I would have started the budesonide though, and worked around the diabetes with insulin.

From two oncologists budesonide really has no benefit for lymphoma while prednisolone does.
 
Thanks all for contributing. We were trying to do a diet change alone, actually he was on one last year, but i was still feeding him deli turkey daily as a treat, which is the protein he is intolerant of. I tried to switch him to novel only, but with eveything that has happened to him in the last year, he now only eats Friskies oceanfish, and sometimes mixed grill. I can barely get him to eat dry. Not sure if it teeth issues or nausea related, or IBD pain related. He has a few things going on.

He seems to improve when I give him anti nausea meds, until it doesn't. Ugh. We tried Atopica for a month, and that made him eat a bit better somedays, but overall it made him more nauesous and I think spiked his creatinine levels.

We are going to the specialsists again on the 22nd. He will have dental done and an endoscopy at same time. I've spent over 7000.00 on him since last Sept and just trying to get an answer. They keep telling me none of his issues on their own would cause issues, but together perhaps they are. I'm just at a loss and tored of spending money for no "real" answers.
 
Colitis is basically an inflammation of the colon which is pretty much the same as IBD although IBD can occur through out the digestive tract. Budesonide is used for IBD in lieu of pred when there is a chance of diabetes since it has a lower risk for causing diabetes. It's worth a try before the pred to see how or if it will work. If there is a definite diagnosis of lymphoma then pred along with chlorambucil is usually used.
Glad to see you are seeing the specialist soon. I spent about the same amount of money on my Leo to get answers. I started off though with ultrasounds, x-rays, endoscopies, etc and got a quick diagnosis but these tests are really expensive. He needs ultrasounds yearly to check the intestines and an echo cardiogram because he had a blood clot three years ago. Geez-

Yeah, he was on prednilisone from Sept thru Nov last year, then got diabetes diagnosis week of Thanksgiving and discontinued pred. He started throwing up aroudn 2010 a couple times a week, but never lost appetite, and in fact gained weight. The throwing up slowed a couple years ago to almost nothing, but is constanstly licking himself almost bare on the legs. We were never told to try a novel diet until last year in March. I stupidly was still giving him turkey as it was his fav and part of our routine. Then all of a sudden he stopped eat dry and then stopped eating wet. Then his food needed to liquifed. I kept saying teeth, but they all said they didnt see anyhting. Well he devolped diabetes, was up to 5 unit 2x a day and still in the 400's. Fianlly was sent to a specialsist to do ultrasound to see if there were issues for unregulated diabetes. They saw he had kidney issues and inflammation in GI. Also had a dental that day. That is what cured his diabetes. He has actually gained weight and never had diarrreah unless he eats turkey now. But he is nausoes and reluctant to eat dry. Id rather him not eat dry, but he will eat novel when he does eat dry.

I wonder if the tooth issue alone caused the diabetes. I know the pred didn't help the cause, but we had stopped that 3 months earlier. I am very happy that the specialsits are allowing for dental and endoscopy same time. I really worry that he just was under anesthia in Feb, and the prospect for 2 more times in a short time period. I still don't even want to do 1 more time, he was so miserable last time. I just hope we can finally get on the right track.
 
Neko couldn't take pred due to her heart, budesonide reduced her bowel inflammation by 30% and improved her GI symptoms. It also did not impact her blood sugar, though I have heard it do for at least one cat. The small cell lymphoma group I belong to that has an IM vet participating, suggests budesonide when pred cannot be taken due to heart or diabetes.
 
My newest IM vet explained these scenarios to me:

He'll take bude and he'll improve, but become diabetic.
He'll take bude, he'll remain the same, and will develop diabetes.
He'll take bude, he'll remain the same, and won't develop diabetes.
He'll take bude, he'll improve, and won't don't develop diabtes.

The scary thing to me is that he is improved and better then where he has been at any point in the last year. He purrs more, goes outside, wants to eat, grooms better. He still has other issues. I would love for him to not be nauseas, if that is even what it is at this point. I am sure it is, but I wonder if it teeth related. Or both. I guess my point is what if we put him on bude and he doesnt improve, AND becomes diabetic again. I will be heartbroken. This is such a tough decision.
 
If the budesonide doesn't do anything for him, take him off it, if he becomes diabetic, you know how to handle it and with budesonide out of the picture he could go OTJ again. Frankly though, too many what if's to worry about what might happen.

Thanks, he is my big baby. Love him to pieces. We go for bloodwork tomorrow and endoscopy and dental on the 22nd. I have a feeling already about the endoscopy. I guess if he has to go on bude, and develops diabetes again, i guess we fight through it. I'll do anything to make him happy and live a longer life. Good look with your kitty. Crossing my fingers the diabetes goes in to remission soon now!
