Jason: Young diabetic cat with DKA, pancreatitis, fatty liver

    Hi everyone.
    I need help. I don't know what else to do.
    My cousin's cat Jason (note that me and my cousin are very involved in each others life, like sisters, and we help each other with out pets, so Jason is like my own) is very sick and he's only 5 years old.
    Jaosn is an albino cat and was diagnosed with diabetes a month ago... Unfortunately my cousin trusted his vet who thought it was a good idea to sell the most expensive kibble available in the market and try to control his diabetes through food changes. Jason didn't get any better because he wasn't producing insulin. He lost weight because he refused to eat that disgusting food and his cells were starving for glucose. When my cousin took him to the vet again, she insisted on the dietary change and once again she hasn't given him insulin. Prior to diagnosis his glucose levels were at 400-500 and on the new diet it went down to 200-300 (only because he was starving). She also noticed fatty liver disease. She sent him home with antibiotics. And no insulin. He started smelling like acetone and I realised he had DKA. He was dying. My aunt took him to the ER and they admitted him with a very poor prognosis. He turned yellow very quickly but they said his pancreas was okay. He was unresponsive to treatment, hasn't gotten any better... Now they found out that his pancreas is not okay afterall and justified that it's hard to diagnose. His bilirubin levels keep rising. He's super yellow, in pain, but he is a little bit more responsive even with his bloodwork indicating the opposite. He's being tubefed, has iv fluids with potassium, he's had a plasma transfusion, he's on antibiotics, methadone, rapid acting insulin... His ketones are gone, his anemia improved, his glicemia is almost under control as well but his liver is only getting worse and he's more yellow day by day. Its so sad to see what once was such a healthy, chubby cat with a pink tone under his beautiful white coat now a yellow bag of bones... They're starting a low dose corticosteroids treatment to see if the liver improves but doctors are saying it might not be worth it... But he's a fighter... I can't describe his strength going through all this and still raising his head to greet us, or waving the tip of his tail whenever we say his name... He was peeing and pooping all over himself (his coat is dyed orange because of the orange urine) and he even ASKED for help when he wanted to pee so we could hold him and help him stand to pee. He doesn't deserve all this please someone suggest something I don't think the doctors are trying hard enough... I think they're unexperienced with feline diabetes...
    I'm attaching 2 up to date photos and 1 (the one where he's laying on my back) when he was healthy. Keep in mind that he is a very big cat. I'm a tall lady and my back is quite spacious and he still managed to cover the entire area with his body. I miss seeing him like that...

    Hi Barbara and welcome to FDMB to you, your cousin and Jason.
    I am so sorry you are having such a bad time with Jason. That is unforgivable that the vet did not start insulin and kept him on kibble.
    It sounds as if he is getting the proper care now thank goodness and the DKA is improving.
    Pancreatitis and Diabetes are both manageable diseases so please take heart with that.
    I have no experience with fatty liver disease but many others do and I'm sure they will chime in.

    Once you get him home there is still a lot you can do to help him recover further and back to normal. When he is closer to that time we can help you get organised with things to support his recovery.
    He sounds like a real fighter so never underestimate that.
    I'm going to tag a couple of people who may be able to help you
    @Marje and Gracie who may be able to help you with questions about lever results etc
    @AliceMeowliss&Cassandra who had a very ill kitty with DKA and who is now back to normal.
    @Penelope and Mačka whose kitty was very ill with DKA and recovered.

    Welcome Barbara, I'm glad you found FDMB. One question, is your cousin going to be able to register and get information and ask questions first hand? It can be difficult to relay information, especially if it's a critical situation.

    Please tell her about FDMB and perhaps she can take a look at your thread here and also become part of the community.

    Hi, thanks for bringing a little bit of hope... Unfortunately it's being complicated because he isn't recovering, he keeps getting worse and vets don't have many hopes, we even had one of the doctors telling us it probably was a waste of money so we shouldnt get our hopes up... My cousin isn't eating or sleeping, she is blaming herself so hard, but it was that vet's fault...
    The new vet team isn't giving us much hope... It hurts

    My cousin is not at her best state of mind because of everything going on. I'm being responsible on all the medical stuff and she is getting sicker and sicker... Maybe if things improve I'll suggest her to join this group so she can learn how to deal with him at home. But now the situation is critical and she even asks me to speak to his doctors because she has stopped eating and sleeping... I'm also attending college, studying veterinarian nursing (freshman year) so I'll be taking care of the hardest parts of Jason's condition...

    I know it is very hard to hear news like that.
    Recently we had a kitty on here that was very ill with a very bad case of pancreatitis as the vets did not hold much hope for recovery but they kept treating him and he amazed everyone at the ER by recovering fully.
    So as long as Jason wants to fight, keep treating him..

    Understand, and it's good that you are on the ball and helping her. I feel so badly for her. I just meant when Jason is well enough to come home and have care there. :bighug::bighug:

    Yes absolutely reading it and living it are two different things.
    So at the moment his liver seems to be the main problem is that correct?

    Yeah I'm trying to convince everyone that if he's still breathing, he's a fighter. He had DKA for at least 3 days. It kills a human in 24h or less, and it kills cats in half a day. He's a champ. Worth every penny and we don't even question that. What concerns me is if we are just prolonging his suffering... Cuz some of his vets are saying the inevitable is going to happen and there's nothing else they can do so we should consider putting him down... But I can tell he doesn't want to die. He's fighting so hard not to drift into a coma. He's very strong...

    When I needed her she was always there. She is the sister I never had. I was very sick with lupus and many complications the disease brought and she was there making sure I didn't give up. My cat was also sick with bowel obstructions because of his long hair and she woke up at 3, 4 even 6 am to ride us to the ER. She saved me and she saved him and I wish I could do the same for her and Jason... Also she's dealing with a sick 10 year old chilchilla at the same time... It's being harsh on her, she got out of a long term relationship, quit her job, left her parents house and she's starting college at 29 years old and this all happened in less than a month. She's in a mental breakdown, it hurts that I can't do more...

    Does he seem in pain? Aren't they addressing that issue?
    I find vets are sometimes too quick to say euthanasia so if you think he is still fighting then I would not agree with putting him down.

    Yes, the main concern is the liver, his bilirrubin levels only keep going up. But the vets are also worried about his pancreas.. Its also being extra hard because some doctors give us hope while others cut us short and tell us to give up... Some talk about his pancreas and even told us his liver was under control while others say the opposite. I call them every morning when I wake up and every night before bed. Sometimes I wake up in the middle of the night and ring them to make sure everything is okay and they tell me he is stable but when I ask something more specific they have to check who he is. Sounds like no one really cares about him but maybe I'm biased and tired as well...

    He is not feeling good and that's visible. But here's what we're clinging to:
    He's young.
    He has never went into a coma and that's against all odds because his blood literally would taste like lemon juice and sugar when he got admitted.
    He tried hard to react to out visits (we all visit him everyday and we always leave late when they start kicking us out).
    Even though his liver keeps getting worse, he only shows more will day by day, he looks at us, he lifts his head, sometimes he tries to stand for us, he meowed in protest of being away from us, he looks at his butterfly toy, he makes the effort not to pee on himself... His strength is unbelievable and no one understands how is that even possible...

    My friend who had the kitty with the really bad pancreatitis was told to put him down. She rang me distraught. she left that vet and found an ER where they were willing to help. At times it seemed futile but he pulled through and is now home and well.
    I would go in and talk to them, make notes. It sounds as if you understand a lot of how it all works so that is an advantage.
    Talk to the vets who are more positive and go from there.
    Just say putting him down is not an option at the moment and please keep treating him..

    Have they tried milk thistle or denamarin to help the liver?

    I don't let them make a fool out of me but then they go and ring my cousin and tell her he's condition is worse and he won't make it and she just collapses and won't get out of her bed, she cries everyday and refuses to eat and I'm losing my mind with those unreliable so called professionals who are making us go through a rollercoaster of emotions, they give us hope and tear it apart everyday.
    It's the best vet hospital in town. My uncle payed aprox 2000usd already and we are willing to pay a lot more even if we lose all our money. That cat is worth it. My cousin's wellbeing is worth it as well. But for some reason, they delayed every step of the treatment whenever I suggested it. When he was admited I told everyone to give him potassium and 0.9 iv. They gave him 0.45 iv and no electrolytes. He got a lot worse for the next 3 days and then they decided to treat him. Then there was the plasma transfusion, I suggested since day 1 or 2, I even offered my cat who is currently an active blood donor and they said they didn't need to do that. Yesterday morning I talked about it again and they told me on the phone that they were not going to need it unless his anemia dropped to 13%. I got there 45min after the call and found out he already had done his blood transfusion, and that the authorization and responsability papers were signed the previous night. Its like they're not even talking about the same cat. It's frustrating

    Yes it does sound frustrating.
    If your cousin is so distressed, would she let you take over the authority for care at moment, until she is better,and then they would have to ring you.....just a thought.
    Can you ask for a plan of the next steps in treatment and when they are going to be implemented?

    It will give her more anxiety if she doesn't speak to them... Whenever they're quiet she's making scenarios about his death... But I told 2 doctors already how to approach my cousin so the medical stuff is mostly on my side.

    It sounds as if you are doing all you can to support your cousin through this.
    I swear by milk thistle........I hope they agree to use one of the two. There would be no harm in trying..

    Do you have his labs and can you post them, please?

    What diagnoses are they giving you for the liver....fatty liver, triaditis, cholangiohepatitis? Have they done an ultrasound to see if he has a bile duct obstruction? High bilirubin can be from fatty liver, bile duct obstruction, pancreatitis, gall bladder inflammation, to name a few.

    Have they done a specfPL for pancreatitis? I didn’t see where they are giving him pain meds.

    I would agree that they should be giving him milk thistle. It’s really wonderful and can help support the liver. The liver is amazing because it can regenerate itself like no other organ if given the right support.

    Labs would really help esp if you can post them as they’ve been taken over time.

    I'll ask for the lab results first thing tomorrow and post a picture here. The initial diagnose was fatty liver combined with pancreatitis but now they don't give us any specific answer to anything anymore... Its like they're afraid to show us... His results are always hidden unlike the other admitted pets who have their files hanging from their cages. Shady.

    Thank you. They should be giving you their daily assessments. If you don’t feel they are being forthcoming, perhaps speak with the clinic owner.

    It can take a lot of time to recover just from fatty liver; add pancreatitis and it’s even longer.

    Any labs or reports you can post will help.

    @Barbara Mota Pinto I'm so glad to see you posting over here!

    @Marje and Gracie is the best at evaluating labwork so please do post Jason's labs ASAP. Don't let them buffalo you...If the bill has been paid, the labwork belongs to you.

    One of my diabetics had both pancreatitis and hepatic lipidosis (fatty liver) at the same time. Wendall was very ill - weak, nauseous, vomiting, hiding, refusing food - and required intensive care if he were to survive. He probably should have been hospitalized but my vet knew that I would devote myself to him so I treated him at home with sub-q fluids, syringe feeding, Denamarin, B-12 shots, Cerenia, Ondansetron, Buprenex and probably other things that I can't remember. Caring for him was pretty much a full time job and he didn't always appreciate it. I don't remember exactly how long it took for him to get better, several weeks anyway, but he did recover. Hoping for the same for Jason.

    How is Jason doing? I can imagine your frustration. His story is so heartbreaking. Sending healing energy to Jason and your cousin and strength to you.

    I can't offer any medical advice. Here are couple things that may help Jason's spirits. If you have not done this already, bring something that has your cousin's scent on it like a shirt or pillowcase. So even if it too much for your cousin to visit him, he will still have her scent to comfort him. If possible visit with him at the clinic as much as possible. That can also help.

    We visit him everyday until they kick us out. Hospital doesnt allow us to take blankets or any other textiles, I've suggested that as well

    What about a bed he loves? I’ve had numerous cats in the hospitals over the years and they’ve always let us bring their bed for them to lay in because it has their and their home’s smells. It’s a comfort and helps with stress. I’d ask again and explain why. If they try to tell you they can test him better without one, that’s just them being lazy.

    Obviously, they won’t want something that lets him hide where they have to struggle and stress him more to check on him but there should be a compromise that gives him comfort but allows them easy access to him.

    Hi @Barbara Mota Pinto, I am just catching up with this thread and am so, so sorry about what's happening to Jason. This is really sad, and the behavior of the staff at the clinic sounds terribly frustrating. Yes, you need to stay on top of everything, know the answers to your questions if possible, and don't hesitate to harass them. When Macka was DKA at the ER clinic, most of the time I was only talking to a receptionist and like with you, they had ready-made answers but did not really know which cat they were talking about. And they also would not inquire to the medical staff unless I insist like a crazy woman. I would go there twice a day, and would not let go.

    You said he was tube fed, is he interested in food at all? I am sorry but I don't have any experience with fatty liver, only DKA and pancreatitis, and yes, they can survive it with intensive care. It's become my full time job though, 24 hours/24, but I wouldn't do it differently.
    Does he receive pain-killers? Anti-nausea meds?

    Oh no... I am so sorry. Just absolutely devastating. When they're so young and have so much fight in them, it's hard to imagine that their bodies could fail them, but sometimes the damage is too severe.

    I am so sorry to hear that he did not make it. It sounds like the little guy just had too many issues to deal with. :(

    I'm so sorry. Jason was a true fighter, but he seemed to have so many health issues going on. He was such a pretty cat and was loved very much.

    After finally being able to talk to Jason's mom, I have something heartbreaking but bittersweet to share with you.
    Jason died because his heart stopped twice and that's all I knew, no further details.
    But knowing the whole story brought tears to my eyes. Jason was supposed to die the day he was taken to the hospital, he had already chosen a place. But he kept fighting. He fought to the last minute to make sure he saw everyone before he died.
    My cousin broke up with her boyfriend, they started a relationship soon after Jason was adopted. They broke up about a month ago. Her ex boyfriend loved Jason, and Jason loved him back. Even though her ex is still suffering from the breakup, he set his heart aside and visited Jason 5 or 6 times during his final week. My mom (who is not that into cats) CRIED for Jason because she, who had made mistakes in the past, was forgiven and then she was invited to have lunch at my family's place to meet the young kitten. And he loved her so much, he massaged her liver when her health was bad, and she wouldn't pay much attention to him but he was persistent. And funny, oh lord he was such a clown. He waited for her visit as well, to say goodbye. My uncle and my aunt (they're my godparents) aren't even friends. Their relationship and love died a long time ago and they can't stand each other. But guess who also brought them together... Their son who is my oldest cousin, used to live at his parents as well but he moved on with his life, went to live with his beautiful woman and 3 weeks ago they had a healthy baby named Andre. He waited for them as well.
    I've been sick since my middle teens with lupus, struggled a lot but I'm finally in remission, and I got into college and started a new relationship. When I was sick Jason would cover me with his blanket and watch me sleep. He waited for my success.     After a week at the vet hospital, Jason had all his visits and knew everyone had reached their personal milestones. He waited for everyone to say goodbye.
    Here comes the bitter part so grab your tissues...
    On his last day I went to visit him at 3h30pm and everyone was late so I was his first visit. We had gotten good news earlier on the phone about his glucose levels stabilising and his bilirubin going down, but he was tired... I wasn't supposed to visit him that day because I was heading to my new school. Something told me I should go. He only moved his eyes to greet me that day. His drool was thick and covering his nostrils and mouth, so I cleaned it up so he could breathe more comfortably. He had an accident and defecated himself. I picked him up to help the vet intern clean him and his cage, and he layed his head on my neck, and grabbed my shoulders with his nails. Sort of felt like a hug, so I hugged him back. I left at 4h51pm (had to check that because I called my godmother in that exact moment, she was still late but I had to leave so he was alone until she arrived. His heart stopped at 5pm. This gives me chills. They were able to reanimate him and called my family and rushed them over, they got there and he raised his head to greet them, his feet were cold because of his blood pressure and all the cleaning products from earlier. They grabbed his former pink paws that were now completely yellow and tried to heat them. He stabilised and they were there for over two hours. Then the nurse kicked them out at 8pm because visiting hours ended at 6pm. They said goodbye, turned him over so he wouldn't develope bed sores and he panicked. My cousin started screaming and crying, my aunt called for help and as the vets took him, they told Jason it was okay for him to go. After many attempts to bring him back, he was already crossing the rainbow bridge. Rest in peace dear soul.

    Goodbye Jason, thank you for everything you did for our family. You did well. We all love you very much.

    That's an amazing story Barbara.

    It's obvious Jason had a mission and until it was complete, he was going to continue to fight. Once he'd completed it, he returned to the Master that had created him and sent him to your family to heal old wounds and bring you together again.

    Quite a feat for a little cat but it sounds like he did his job well and now he's reaping his reward. The best places to chase butterflies, the sunniest places to nap and surrounded by all the love his heart could desire. Soon (for him, since time passes differently at the Bridge) his people will join him and together you'll all cross the bridge and enter Heaven together...forever.
