I looked at Jessica's labs from yesterday. Her kidney values aren't elevated in the way one would normally see if there is a kidney infection. I'm not saying she doesn't have one; but typically you would see an elevated creatinine. When you divide the BUN by the BUN/Creat ratio, you get a creatinine of 1.0 which is totally normal. Her BUN could be elevated from just a bit of dehydration or from a high protein diet. I do think the elevated WBCs indicate something is going on. Typically you would need an ultrasound to confirm a kidney infection. A kidney infection might make them feel bad but my Gussie has had one three times and I never could tell clinically that there was anything wrong.

I wonder why the vet wants to do a B12 test? Is she talking about cobalamin?

I didn't see a condo yet today so I'll post this here. In response to your PM asking me to look at Jessica's labs and U/S findings, I think her labs just show her to be a little dehydrated but she does have a high white cell count indicating some infection.

The U/S also indicated urinary tract changes. I thought it was interesting the vet did not fully discuss how the kidneys looked....were they hyperechoic indicating a kidney infection? I think I would seriously discuss with my vet that a long term antibiotic might be needed here to address a possible kidney infection. Typically, 4-6 weeks with baytril is needed. As I've indicated before, just because the culture and sensitivity did not grow bacteria does not mean a kidney infection might not be present. Gus has never had bacteria on the C&S but has had three kidney infections.

I'm also concerned about the intestinal findings. It's also odd he did not say any more about that other than "gastrointestinal disease". I would be concerned about the nodules on the liver and also the changes to the lymph nodes as well as thickening of the intestines. I'd discuss with my vet the possibility of inflammatory bowel disease but also lymphoma.

There is something going on and they need to find it. They might suggest exploratory surgery....I'm not a big fan of that but it's your decision.
