Re: 5/15 Topper AMPS 229

I've reviewed your posts from the start. You have been doing a great job working around the limitations and have been careful to make small dose increases as they were warranted.

As your doses have crept upward, you may be concerned that they are very high; you did the increases in small steps after sufficient waiting which makes it much less likely that you've passed an optimal dose.

There are a few high dose conditions which exist - acromegaly, Cushings, and insulin autoantibodies (IAA). There are specific tests for acro and IAA; Cushings is more difficult to diagnose. We have a sub-forum here with more detailed information on the conditions.

Could it be possible that the dose is higher because he went for several months after being diagnosed, with the dose being raised too slowly to really bring the bg down?
Am I making sense?

Hi Jan, welcome to LantusLand! It's a great place, I'm sorry you and Topper are here.   It is the best place for getting advice on treating Topper, that's for sure. His Spreadsheet is looking better and better, you are doing a great job with managing everything.

Jan

He is not at a high dose right now. We don't start thinking about high dose conditions until they get over 6u bid. Dont worry about IAA or acro for now. He's likely to need more insulin to get into normal numbers routinely but he's looking good.

And yes, the longer they stay at high numbers like he did before you came to FDMB, the more insulin it takes to reach a breakthrough dose.

You are doing great!
