I wanted to test him much sooner than +3.25 but I guess I lucked out with the bounce. I went out in to the garage and he thought he was going to the vets and managed to get into the one place in the house where I couldn't get to him (under a real heavy bed that's in the corner of a room up against the wall). Little stinker...

Got a call from the internist today with very encouraging results from yesterday's blood work. His ALT dropped from 722 to 38 and the Spec fPL went from 25 to 10. Still high and indicative of pancreatitis but possibly less severe. She said the drops could be due to the metronidazole and may be short-lived but to bring him back in a month to re-test.

She did recommend getting him off the buprenorphine as long as he seemed fine. Her concern is that animals can get addicted to it just like humans, and an appearance of feeling fine and happy could be just a side effect of the narcotic. She also suggested getting him off fluids. She said that if he didn't have diabetes, she would see no harm in giving him fluids. But although there are no studies or literature to support insulin malabsorption due to Sub-q fluids, she said to her, logically, it makes sense that's it's a possibility. She also supported this with his urinalysis results last week that did not indicate dehydration despite all the vomiting that had been occurring prior.

I can't think of anyone else right now, but would appreciate thoughts on what she said. Particularly, discontinuing the fluids and bupe.

I would run everything by the pancreatitis group if Jinx were my cat. And of course - flag that you have a question for the experts here about fluids. Maybe you could forgo them, do you know how to check if Jinx is hydrated?

Hi Karrie, I'm going to post on the pancreatitis board in a bit. As far as checking whether he's dehydrated... I know tacky gums are an indicator but it's really tough to get Jinx to let me do that. I haven't tried while he's sleeping yet but will give it a shot tonight. But if I remember right, I think Marje might have posted before that checking while or after they have been sleeping is not the best time. I could be totally wrong about that. I am aware of the skin turgor test but that doesn't strike me as being 100% reliable. Any advice/suggestions would be appreciated. I'll also pose the question to the pancreatitis group.

I think I'm just going to have to figure out a way to get him to better tolerate me touching his gums.

Forgot to mention that weighing him before and after fluids and noting the drop in fluid weight is another option.

That's another option to tell if he's hydrated? Interesting. How does that work?

J.D. drinks a lot of water. I've felt his gums, and they seem okay. I've pulled the skin behind his head and it goes back down pretty fast but not instantly, so I'm not sure how to test so much for hydration myself, so will be interested in any replys you get. Of course, I should go read the Feline Pancreatic Support Group emails, too, there's just so many of them, I'm almost always overwhelmed and can't keep up.

ALT is withing normal range, now? That is great :thumbup

Actually, I don't know what I was thinking... :roll: :roll: That's a way to determine if they've absorbed the fluids.
